Community Rehabilitation Centre

The Community Rehabilitation Centre (CRC) provides interdisciplinary rehabilitation services and community care to the shires of Mount Alexander, Macedon Ranges and Central Goldfields.

People can attend for specialised programs or individual treatment provided by a range of health practitioners. A small fee applies however may be reduced or waived if payment will cause hardship.

Referrals are accepted from Rehabilitation or Acute units, General Practitioners, Health Professionals, other health and community facilities, clients or their families.

*Only referrals for physiotherapy need to come from a GP.

 

 

 

Allied Health services

Specialist staff at the Community Rehabilitation Centre aim to deliver services to meet the individual needs and goals of each client.

Referrals are welcome from any source, including self-referral. Referrals for allied health services sometimes require prioritising and sometimes there is a waiting list. People whose disability places them in a vulnerable situation or who are at risk and cannot access service elsewhere have highest priority. For more information about waiting times, please speak to the intake workers or reception staff.

If you are unable to attend an appointment at the centre, please speak to the intake workers as appointments at your home can be arranged for eligible clients.

How to refer to CRC Services

Castlemaine Health primary care and subacute ambulatory services require referrals to be:

  • transmitted using eReferral via ConnectingCare
  • posted using the Department of Human Services’ state-wide Service Coordination Tool Templates (referred to as the “SCTT”)
  • General Practitioners may use the similar “State-wide Referral Tool”
  • Services within Castlemaine Health may refer to other Castlemaine Health services using these tools via the secure eReferral System, ConnectingCare

The tool templates required for a referral are:

  • Referral cover sheet (or secure eReferral system)
  • Consumer information
  • Summary and referral

The “Consumer Consent to Share Information” form should also be completed and retained by the referring service.

Additionally, referrals from other health and community services should also include:

  • Current medications
  • Medical conditions
  • Clinic/service required
