Enquiries and Referrals

Castlemaine Health primary care and subacute ambulatory services prefers referrals to be transmitted using eReferral via ConnectingCare or faxed or posted using the Department of Human Services’ state-wide Service Coordination Tool Templates (referred to as the “SCTT”). General Practitioners may use the similar “State-wide Referral Tool”.

The tool templates required for a referral are:
• Referral cover sheet (or secure eReferral system)
• Consumer information
• Summary and referral information

Summary and referral Referrals for consumers with complex needs or chronic conditions should also include:
• Functional profile
• Living and caring arrangements profile

Referrals are processed by intake workers and are an important communication tool. The information on the referral is essential for our staff in triaging referrals and managing waiting lists effectively. On all referrals, please clearly state:
• The purpose of the referral/ clinic service required
• Current medications
• Medical conditions
• Clinic/service required
• Accurate client and referrer contact details

The “Consumer Consent to Share Information” form should also be completed and retained by the referring service.
