About the role
This role
We are seeking expressions of interest for a Nursing Care Coordinator for our HARP/Complex Care team. You will work with clients with chronic health diseases, complex psychosocial and complex aged needs. The role is fixed term covering long service leave (22/5/2023 to 3/10/2023). It would be the ideal opportunity to undertake a role in the team as ongoing hours will be likely moving forwards.
The role, in alignment with the Health Independence (HIP) guidelines provides short term support to eligible people and their carers to:
• Improve patient outcomes;
• Provide integrated seamless care within and across hospital and community sectors;
• Reduce avoidable hospital admissions and emergency department presentations and;
• Ensure equitable access to health care.
About you
• Bachelor of Nursing with current AHPRA registration
• At least 3 years full time equivalent working as registered nurse
• Extensive social model of care experience in dealing with complex clients is essential
• Current Victorian drivers’ licence
