Community Programs

Castlemaine Health provides a number of community services. These are:

  • Adult Day Services
  • District Nursing Service
  • Outpatient Rehabilitation Services
  • Post-Acute Care.

Adult Day Service

The Adult Day Service (ADS) provides planned activity groups, respite for Carers. The ADS team also run Castlemaine Health’s volunteer program. The volunteer program matches women and men of all ages and backgrounds to roles at Castlemaine Health and in the community. Through these roles, volunteers help local people maintain vital social connections, access health and medical services, and take part in health and wellbeing activities.

District Nursing Service

The District Nursing Service provides general district nursing, hospital in the home and palliative care to patients across most of the catchment area of the local shire.

Outpatient Rehabilitation Services

At Castlemaine Health, outpatient rehabilitation services are delivered through the Community Rehabilitation Centre (CRC). The CRC offers interdisciplinary rehabilitation services and community care to the shires of Mount Alexander, Macedon Ranges and Goldfields.

Post Acute Care

Post Acute Care is the management and provision of short term, planned care. It’s provided for for patients with complex needs during their recovery from an episode of acute illness or injury.
